SOC/Admission Visits: This pharmacy requires their own consent form. This must be completed during the initial/admission visit (along with the HHC admission paperwork and consent). The form can be found in the Duke Home Infusion folder sent with the patient's supplies.

  • The nurse (you) needs to sign the form as a witness, and make sure the patient signs the bottom and writes the correct name and DOB at the top.

  • If the patient claims they have already signed this form, that's great. We still need a copy. We cannot provide service until the form is signed, so the patient cannot refuse or wait to sign and mail after the visit.

  • Please take a clear picture of the entire form (even a pink or yellow copy) with your phone and text it to your VNs & Pay thread in Whatsapp.

Clinical Considerations:

  • Will not dispense/administer Cathflo in the home

  • Solumedrol is generally dispensed in a medi/accuflo ball

  • IVIG Standard: Vented spike, infused directly from bottle

SOC/Admission Visits: A medication profile (list of all current medications) is to be completed with the admission paperwork (5 pages total). There should also be a folder at the patient's home which includes additional form(s) to be completed by the RN.  Please send these form(s) with your visit notes (instead of sending directly to the Pharmacy).  Lastly, there will be a "Financial Obligation" form.  Have the patient sign, place in the pre-stamped envelope, and mail back to the Pharmacy.

  • IVIG Standard: Pooling bag, tubing is primed by pharmacy

  • TPN: Daily CAP changes for pediatrics patients when infusing TPN (teach family)

    • Chorhexidine swabs, 2 masks, 1 set of gloves, or

    • Alcohol/betadine sticks, 1 mask, 1 set of gloves

    • Biopatch: only if ordered by MD

SOC/Admission Visits: In addition to the standard HHC 4-page admission paperwork, there will be a 15-20 page Welcome packet sent by the pharmacy with the meds/supplies. Please carefully look over each sheet, fill it out and have the patient sign as applicable. Send this paperwork in with your visit notes and a supply form if any supplies are needed for the next infusion.

Post Infusion Call Requirement:

**Please make this call prior to leaving the patient's home and include this time in your visit. Please note on your visit documentation that this call was completed and the name of the person you reported to (or if you left a voicemail).

    • Option 1 for Nursing Coordinator Voicemail, or

    • Option 2 to speak with a Nursing Coordinator

  • Verbal Report should contain the following:

    • Patient Name

    • Patient Date of Birth

    • Infusion Date

    • Length of Infusion

    • Next Scheduled Visit

    • Brief Summary of Infusion noting any issues, adverse reactions, or improvement of symptoms

 

  • If a scheduled date has been cancelled or rescheduled, please provide:

    • Patient Name

    • Patient Date of Birth

    • Date Infusion was Scheduled

    • Reason for cancellation or reschedule

    • Rescheduled date(s) if applicable
