Basic Step-by-Step: 

 

**ALWAYS establish patent IV access and flush PRIOR to mixing medication**

MIXING

  • Obtain a clear/clean space to prepare your bag of Remicade.

  • Pharmacy will provide syringes and needles to prepare medication. Be sure all the necessary supplies are available to complete infusion from beginning to end. 

  • You will need IV start supplies, syringe, needle, and sterile water for reconstitution, bag of NS, tubing (gravity tubing or pump tubing if using a pump).

  • Reconstitute each 100mg vial with 10ml of Normal Saline.

  • Swirl gently to mix.

  • Add Remicade to Normal Saline bag and mix gently.


ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Remicade & set med up on pump to infuse over 2 hours.

  3. Monitor patient for any adverse reactions.  Take vitals one hour into infusion and record.

  4. Record vitals every 15 minutes for the 1st hour, then hourly thereafter.

  5. Record vitals 5 minutes post infusion.

  6. Record lot numbers and expiration dates for vials used.

  7. Disconnect patient, flush line, and remove IV. Place pressure bandage at site. 

  8. Clean and dispose of used supplies.

MIXING

  • Obtain a clear/clean space to prepare your bag of Actemra.

  • Pharmacy provides needles and 60mL syringes to prepare medication.

  • Be sure all the necessary supplies are available to complete infusion from beginning to end.  You will need IV start supplies, syringe, needle,  bag of NS, tubing (gravity tubing or pump tubing if using a pump).

  • Determine the volume of Actemra to be infused. Remove and discard that volume of saline from the saline bag (normally 100mL bag). For example, if the ordered dose of Actemra is 24mL, then remove 24mL of saline from bag.

  • Draw up Actemra from the vial(s) to get the ordered dose.  (Actemra is weight based and therefore dose will vary patient to patient.)

  • Add Actemra to your saline bag and mix gently.


ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Actemra & prime line.

  3. Infuse Actemra over 1 hour or per order.

  4. Record vitals 15 minutes after infusion start and 5 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. Disconnect patient, flush line, and remove IV. Place pressure bandage at site. 

  7. Clean and dispose of used supplies.

MIXING

  • Obtain a clear/clean space to prepare the Entyvio.

  • Be sure all the necessary supplies are available to complete infusion from beginning to end.  You will need IV start supplies, syringe, needle, and sterile water for reconstitution, 250ml bag of NS, tubing (gravity tubing or pump tubing if using a pump).

  • Entyvio comes as a powder in a glass vial. Reconstitute powder by adding 4.8ml of sterile water to the vial. 

  • Direct the sterile water stream to the wall of the vial to avoid excessive foaming.

  • Gently swirl to mix.  Do not shake. Allow powder to fully dissolve.

  • Withdraw the 5ml reconstituted Entyvio solution from the vial and add to the bag of Normal Saline.

  • Gently mix the bag.


ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Entyvio & prime line.

  3. Infuse Entyvio over 30 minutes or per order.

  4. Record vitals 15 minutes after infusion start and 5 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. Disconnect patient, flush line, and remove IV. Place pressure bandage at site. 

  7. Clean and dispose of used supplies.

Cimzia

Basic Step-by-Step:   

 

**ALWAYS establish patent IV access and flush PRIOR to mixing medication**

MIXING

  • DO NOT mix/add the meds into the bag of saline! The bag of saline is for pulling a desired amount to dilute the med down to 5mg/ml.

  • Pharmacy provides needles and 60ml syringes to draw up the meds and saline.

  • Soliris is prescribed in 600, 900 or 1200mg doses infused over 35min.

  • You will have vials of 300mg in 30ml (10mg/ml). You will draw up 2, 3, or 4 vials to get the ordered dose.

  • The mix ratio for this drug is 5mg/ml so NS is used to dilute the meds.

    • 600mg Dose = 2 vials = 60ml (so you need to dilute with 60ml of NS)

    • 900mg Dose = 3 vials = 90ml (so you need to dilute with 90ml of NS)

    • 1200mg Dose= 4 vials = 120ml (so you need to dilute with 120ml of NS)

  • Based on the calculations above draw up the needed amount of NS from the bag (60, 90 or 120ml).

    • Place the diluant into the empty bag. Discard any leftover saline.

  • Draw up the meds (2, 3 or 4 vials) based on the ordered dose (600, 900 or 1200mg).

  • Add the meds to the bag with the NS. Mix gently.

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Soliris & prime line.

  3. The pump should be pre-programmed by the pharmacy to infuse the total volume (120, 180 or 240ml) over 35 minutes.

  4. Record vitals 15 minutes after infusion start and 5 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. Disconnect patient, flush line, and remove IV. Place pressure bandage at site. 

  7. Clean and dispose of used supplies.

MIXING

  • Obtain a clear/clean space to prepare the Tysabri.

  • Be sure all the necessary supplies are available to complete infusion from beginning to end.  You will need IV start supplies, syringe, needle, and a bag of NS (usually 100ml), tubing (gravity tubing or pump tubing if using a pump).

  • Inspect the Tysabri vial for particulate material and discoloration prior to dilution.

  • Withdraw 15ml of Tysabri from the vial using a sterile needle and syringe.

  • Inject the concentrate into the 100ml bag of Normal Saline.

  • Gently invert the Tysabri solution to mix completely. Do not shake.

  • Allow solution to warm to room temperature.

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Tysabri & prime line.

  3. The pump should be pre-programmed by the pharmacy to infuse the total volume over approximately 1 hour.

  4. Record vitals every 15 minutes for the hour, then 30 and 60 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. Disconnect patient, flush line, and remove IV. Place pressure bandage at site. 

  7. Clean and dispose of used supplies.
